Please fill out this form when submitting a braid. Please note that while I am asking for your name and contact info, that is for my own records. I plan to use your contact info only if I have a question about your braid, or to keep you informed of braid updates and exhibitions. I will never share your contact info with anyone. You have the choice to have your braid identified by your name, or for it to remain anonymous.
